Every massage therapy office has some basic forms for you to fill out on your first visit. These forms vary from office to office but are vital for record keeping. Chattanooga Massage & Bodyworks has 3 standard forms we require of each client.
 
The first form is our basic client intake form. This form provides us with your contact information. It also will provide your therapist with vital information regarding your current health status, pain levels, and other things that can help with your treatment.
 
Our second form is a basic health history form that can alert your therapist to any conditions that may be contra-indicative to massage. It will also inform your therapist of any issues that may need special care or certain techniques to avoid.
 
The third form we have you fill out is our massage therapy consent form. This provides you with information regarding what your therapist can and can not do. It also gives your therapist permission to perform your massage; in other words, you are consenting to allow the therapist to perform the massage. Therapeutic Breast Massage requires an additional informed consent to be signed before therapeutic breast massage can be performed. The breast massage informed consent is located on the bottom half of our regular Informed Consent Form and should only be signed if you are consenting to therapeutic breast massage.
 
If you ever have a change in your address or health, you should notify your therapist so your information can be updated.
